STUDENT INFORMATION:
The following information will be requested during registration. Please be prepared and have all of this information handy when you are ready to register.
"We understand that our current information system does not match
international names, phone numbers, addresses, and other similar data, but
we ask you to do your best with our currect system until we have it
replaced in May, 2005."

- student number - not required to be your Social Security Number. If the student number you already use for school has less than 10 digits, suggest you use the same number and add leading zero's.

- You will need to set up an email account prior to registration. Many students find it useful to set up an email address to devote exclusively to school work. Free accounts are available at yahoo.com, which offers the most storage, and also hotmail.com. Email from FLVS will go directly to this address so it is very important to enter it correctly. In order for FLVS email to reach you, parental controls and any other restrictions on this email account must be disabled. Do not reject any users or only accept mail from certain users on this account, as you may be contacted by several FLVS staff members while you are a student with us. Keep this mailbox cleaned out so your storage allocation is not exceeded. Your personal information including this email address is secure and will not be divulged by FLVS. This email address must be appropriate for school and cannot be offensive in any way - review our student code of conduct on the website if you have any questions about what is appropriate. Please do not make us take valuable school staff time to have you change an unusable email address after you register.

- school name - select after district. Choose the school you will attend during the semester for which you are registering. Make every effort to find your school in the drop-down list to avoid delaying your registration. Choose not listed only after looking very carefully to be certain your school name does not appear there. Select Home Schooled if applicable for the semester for which you are registering.

- school lunch status (optional - used for state demographic data collection)
- school guidance counselor first and last name - contact info optional but extremely helpful in case we need to contact your school to assist you
- counselor working email address (optional)
- counselor phone number including area code (optional)
- counselor fax number including area code (optional)
- grade level (choose the following from drop-down menus)
- current GPA
- current weighted GPA
- projected graduation date
- Do you have an IEP program?
- If yes, which program?
- Are you a Limited English Proficient (LEP) student?
- COURSE SELECTION and COMPLETING REGISTRATION: Decide on courses ahead of time! Discuss with your parent and school guidance counselor. Signature approvals from both are required for enrollment.
